Analysis of Field Data to Describe the Effect of Context (Acoustic and Non-Acoustic Factors) on Urban Soundscapes

    Get PDF
The need to improve acoustic environments in our cities has led to increased interest in correcting or minimising noise pollution in urban environments, something that has been associated with the resurgence of the soundscape approach. This line of research highlights the importance of context in the perception of acoustic environments. Despite this, few studies consider together a wide number of variables relating to the context, and analyse the relative importance of each. The purpose of this paper is therefore to identify the acoustic and non-acoustic characteristics of a place (context) that influence an individual’s perception of the sound environment and the relative importance of these factors in soundscape. The aim is to continue advancing in the definition of an acoustic comfort indicator for urban places. The data used here were collected in various soundscape campaigns carried out by Tecnalia in Bilbao (Spain) between 2011 and 2014. These studies involved 534 evaluations of 10 different places. The results indicate that many diverse contextual factors determine soundscape, the most important being the congruence between soundscape and landscape. The limitations of the findings and suggestions for further research are also discussed. The research presented in this manuscript has been developed with the financial support of the Basque Government, the Bilbao City council, and within the framework of the LIFE QUADMAP project (LIFE 10/ENV/IT/407

This paper reports an example of an urban sustainable regeneration process in a small open square following the soundscape approach in urban design and involving a transdisciplinary team. The renovation of this urban public space brought changes in its acoustic environment and improved soundscape perception as much more pleasant (enhanced comfort). The rehabilitation broadened the use of the square and enhanced users’ wellbeing and health, showing a significant positive impact of sound: reduction in negative emotions and perceived stress while increasing positive feelings. To conclude, the implications of the results about restorative environments in urban sound planning and future areas of research are discussed. This research received no external funding

    Get PDF
This study focuses on the investigation of the mental health of LGBTIQ+ people in school settings, through a systematic review of the literature, using the PRISMA methodology. In the methodological process, both inclusion and exclusion criteria were applied, which are detailed in the flow chart, along with a word map summarizing the findings. From this, an analysis matrix was elaborated that included a total of 25 studies, which revealed three categories of analysis: Suicide risk, School exclusion due to gender diversity and sexual orientation, and Mental health interventions aimed at the LGBTI population. It is concluded that aspects linked to the recognition and expression of gender and diverse sexual orientation should be closely related to the promotion of rights, access to justice, and equity. These aspects should be incorporated mainly in school environments to promote research that is integrated into curricula and focused on peacebuilding. This integration can break the chains of discrimination and contribute to the development of knowledge through a discursive perspective based on a multicultural approach centered on gender, with the objective of strengthening the experience of LGBTIQ+ people

Purpose: Pediatric neuro-oncology resources are mostly unknown in Chile. We report the human and material resources available in Chilean hospitals providing pediatric neuro-oncology services. Methods: A cross-sectional survey was distributed to 17 hospitals providing pediatric neuro-oncology services (Programa Infantil Nacional de Drogas Antineoplásicas [PINDA] hospitals, 11; private, 6). Results: Response rate was 71% (PINDA, 8; private, 4). Pediatric neuro-oncology services were mainly provided within general hospitals (67%). Registries for pediatric CNS tumors and chemotherapy-related toxicities were available in 100% and 67% of hospitals, respectively. CNS tumors were treated by pediatric oncologists in 92% of hospitals; none were formally trained in neuro-oncology. The most used treatment protocols were the national PINDA protocols. All WHO essential medicines for childhood cancer were available in more than 80% of the hospitals except for gemcitabine, oxaliplatin, paclitaxel, and procarbazine. The median number of pediatric neurosurgeons per hospital was two (range, 2-6). General neuroradiologists were available in 83% of the centers. Pathology specimens were sent to neuropathologists (58%), adult pathologists (25%), and pediatric pathologists (17%). Intensity-modulated radiotherapy, conformal radiotherapy, and cobalt radiotherapy were used by 67%, 58%, and 42% of hospitals, respectively. Only one private hospital performed autologous hematopoietic cell transplant for children with CNS tumors. Conclusion: A wide range of up-to-date treatment modalities are available for children with CNS tumors. Our survey highlights future directions to improve the pediatric neuro-oncology services available in Chile such as the expansion of multidisciplinary clinics, palliative care services, long-term cancer survivorship programs, dedicated clinical research support teams, establishing standardized mechanism for sending pathologic specimen for second opinion to international specialized centers, and establishing specialized neuro-oncology training program

Atrasentan and renal events in patients with type 2 diabetes and chronic kidney disease (SONAR): a double-blind, randomised, placebo-controlled trial

Background: Short-term treatment for people with type 2 diabetes using a low dose of the selective endothelin A receptor antagonist atrasentan reduces albuminuria without causing significant sodium retention. We report the long-term effects of treatment with atrasentan on major renal outcomes. Methods: We did this double-blind, randomised, placebo-controlled trial at 689 sites in 41 countries. We enrolled adults aged 18–85 years with type 2 diabetes, estimated glomerular filtration rate (eGFR)25–75 mL/min per 1·73 m 2 of body surface area, and a urine albumin-to-creatinine ratio (UACR)of 300–5000 mg/g who had received maximum labelled or tolerated renin–angiotensin system inhibition for at least 4 weeks. Participants were given atrasentan 0·75 mg orally daily during an enrichment period before random group assignment. Those with a UACR decrease of at least 30% with no substantial fluid retention during the enrichment period (responders)were included in the double-blind treatment period. Responders were randomly assigned to receive either atrasentan 0·75 mg orally daily or placebo. All patients and investigators were masked to treatment assignment. The primary endpoint was a composite of doubling of serum creatinine (sustained for ≥30 days)or end-stage kidney disease (eGFR <15 mL/min per 1·73 m 2 sustained for ≥90 days, chronic dialysis for ≥90 days, kidney transplantation, or death from kidney failure)in the intention-to-treat population of all responders. Safety was assessed in all patients who received at least one dose of their assigned study treatment. The study is registered with ClinicalTrials.gov, number NCT01858532. Findings: Between May 17, 2013, and July 13, 2017, 11 087 patients were screened; 5117 entered the enrichment period, and 4711 completed the enrichment period. Of these, 2648 patients were responders and were randomly assigned to the atrasentan group (n=1325)or placebo group (n=1323). Median follow-up was 2·2 years (IQR 1·4–2·9). 79 (6·0%)of 1325 patients in the atrasentan group and 105 (7·9%)of 1323 in the placebo group had a primary composite renal endpoint event (hazard ratio [HR]0·65 [95% CI 0·49–0·88]; p=0·0047). Fluid retention and anaemia adverse events, which have been previously attributed to endothelin receptor antagonists, were more frequent in the atrasentan group than in the placebo group. Hospital admission for heart failure occurred in 47 (3·5%)of 1325 patients in the atrasentan group and 34 (2·6%)of 1323 patients in the placebo group (HR 1·33 [95% CI 0·85–2·07]; p=0·208). 58 (4·4%)patients in the atrasentan group and 52 (3·9%)in the placebo group died (HR 1·09 [95% CI 0·75–1·59]; p=0·65). Interpretation: Atrasentan reduced the risk of renal events in patients with diabetes and chronic kidney disease who were selected to optimise efficacy and safety. These data support a potential role for selective endothelin receptor antagonists in protecting renal function in patients with type 2 diabetes at high risk of developing end-stage kidney disease.
